Health classes explained
The hierarchy of rate classes: Preferred Plus, Preferred, Standard Plus, and Standard—what defines each, and what factors move applicants between them.
Carriers structure term pricing by rate class. Although carrier names vary slightly, the structure stays constant: Preferred Plus (sometimes Super Preferred), Preferred, Standard Plus, and Standard, with further adjustments for higher-than-standard risk. Those who use tobacco face a distinct set of classes at elevated rates.
What underwriters look at
Physical metrics (height, weight), blood pressure and cholesterol readings, family medical history of cancer or early heart disease, driving record, medication list, job and leisure pursuits, and smoking or nicotine use. The highest classes demand excellent results across nearly every category; Preferred allows modest variations; Standard Plus and Standard accommodate managed conditions such as treated high blood pressure.

Quotes reflect this class; the carrier's final determination occurs after underwriting. If unsure which category fits, get two quotes and compare; the application process makes the final call. Selecting a more optimistic class does not lower your eventual bill; it just makes the estimate less accurate.
